Your first visit to Zebulon Dental Center is the time when Dr. Steven Golubow, Dr. Steve to those who know him, and our team will get to know you and your smile. During this first appointment, we will typically provide an exam which includes an oral cancer screening, soft and hard tissue evaluation, and review of your treatment options. We recommend most of our patients start with a cleaning at this first visit. In emergency situations, we can often complete or begin your treatment on the same day as the consultation. If you have a complex treatment plan or medical history, we may require an evaluation and second appointment to provide the treatment on another day and verify insurance coverage if necessary.
Please note that all patients under the age of 18 must be accompanied by a parent or guardian. If you have a medical condition that may be of concern (e.g., high blood pressure, diabetes, rheumatic fever, artificial heart valves or joints, etc.), or any medications (e.g., anticoagulant therapy, aspirin, heart medications, etc.), please alert the office prior to treatment.
If your previous dentist or physician has taken X-rays, please request that they forward them to our office. If there is not enough time for them to transfer the X-rays, you may pick them up and bring them to our office. In the case that additional X-rays are needed, we will take them at our facility.
New Patient Forms
Please print and fill out the following forms, and bring them with you to your first appointment. We will also provide copies in our office if you are unable print them or forget to bring them with you
.
If you have any questions about these forms, please don't hesitate to give us a call.
Insurance Policy
We will gladly bill participating and non-participating insurance companies on the date services are rendered as a courtesy to our patients. You are expected to pay your deductible(s), estimated co-payment(s) and fees for non-covered services at the time service is rendered. Because Georgia law requires that insurance companies pay claims within 30 days of receipt, any balance still owed to your account 60 days beyond the service date will be billed to you for payment in full.
Our staff will be glad to assist in securing payment on delinquent claims. However, because your insurance policy is ultimately a contract between you and your insurance carrier, and not our office, we cannot negotiate settlement of a denied claim. You are ultimately responsible for all charges incurred in your treatment regardless of insurance coverage.
Although our staff tries to be as helpful as possible in working with you and your dental insurance, it is ultimately the patient’s responsibility to know the coverage and limitations for their dental insurance plan. We will not be responsible for the cost of any dental care rendered that isn’t covered by your insurance policy.
We are pleased to offer Care Credit, which is a payment plan for healthcare services. Care Credit offers special financing and payment plans for our dental procedures. The application process is quick, easy, and can be done during your next appointment. We will be happy to discuss the different plans with you, and assist in the application process. Visit www.carecredit.com to find out more.
