X-rays If your previous dentist or physician has taken X-rays, please request that they forward them to our office. If there is not enough time for them to transfer the X-rays, you may pick them up and bring them to our office. In the case that additional X-rays are needed, we will take them at our facility.

New Patient Forms New patients can fill out forms online or in the office. Just be sure to bring a copy of your insurance card and drivers license with you to your first appointment.

CareCredit We are pleased to offer Care Credit, which is a payment plan for healthcare services. Care Credit offers special financing and payment plans for our dental procedures. The application process is quick, easy, and can be done during your next appointment. We will be happy to discuss the different plans with you, and assist in the application process. Cancellation Policy If you find that you are unable to keep your appointment, please make us aware at least 24 hours in advance so that another patient can fill your spot. Failure to notify us will result in a missed appointment charge.
